Black Crown of Aerdy

This evil headgear was worn by one of the first Overkings of House Naelax in the ancient Great Kingdom. Whether the crown was enchanted by an evil wizard or whether it merely became a receptacle for the Overking's cruel spirit is not known. The powers of the crown exist only to recreate the greatness and wickedness of that Overking. When you attune to this magic item by donning it, your Intelligence and Wisdom score raise by 4 up to a maximum of 24, and you gain one Epic Boon of your choice, even if your level is lower than 19, and your alignment becomes lawful evil. Each time you finish a Long Rest you must make a Wisdom saving throw. On a failed save your Constitution score permanently lowers by 1. When your Constitution score becomes 0, you die and become a specter guarding the crown. After the attunement to this magic item, donning it, you don't voluntarily remove it. The crown can be removed only by a wish spell.
